Medicare and other insurance often maintain certain eligibility requirements to qualify for home health benefits. If you are having difficulty leaving home, or if you are dealing with health issues such as:
- Recent hospitalization
- Orthopaedic aftercare
- A new diagnosis, such as COPD
- Falling frequently
- Swallowing complications

We are proud to have a team of healthcare professionals, including physicians, registered nurses, licensed practical nurses, home health aides, social workers, and physical, occupational, and speech therapists who coordinate care based on an individual’s needs.
A nurse or therapist will make the first visit to assess the client. Your Covenant At Home staff member will contact the patient’s physician with a report. A care plan will be developed by the Covenant at Home care team, along with the patient and family.
Medicare and many insurances require patients to be “homebound” to qualify for home health services. That means it is difficult for you to leave home. It does not mean you are bedbound or can never leave your home. If you leave home infrequently because it requires so much effort, you may meet the homebound requirement. You can call us at (989)799-6020 and ask for the intake department to clarify your homebound status.
Most Covenant at Home services are certified for reimbursement through Medicare and Medicaid. Covenant at Home participates with most commercial insurers and HMOs and accepts private payments.
Services are provided regardless of age, race, gender, sexual preference, religion, national origin, or ability to pay. Financial assistance is available to qualified uninsured and underinsured patients.
